Plan A
$1000 per month for accidents only on or off the job. Benefit is payable for up to twelve months. Coverage starts on the 15th day.
Plan A has no illness or sickness coverage.
Plan "A" is funded by the HPOU bargaining contract.
Plan B
Accidents $1000 per month for accidents on or off the job. Benefit is payable for up to twelve months. Coverage starts on the 15th day.
Illness or sickness coverage provides $1000 per month. Benefit is payable for up to six months. Coverage starts on the 15th day.
Plan C
Accidents $1500 per month for accidents on or off the job. Benefit is payable for up to twelve months. Coverage starts on the 15th day.
Illness or sickness coverage provides $1500 per month. Benefit is payable for up to twelve months. Coverage starts on the 15th day.
